TAPNET Participant Important Forms

 The following forms are used to assist participants going through peer assistance in maintaining communication with TAPNET and to facilitate communication between TAPNET and employers and health care providers. These forms are available for download by participants, advocates and employers.

This is not an all-inclusive list of forms.  Please consult with your assigned case manager/advocate to ensure that you have all necessary documentation for program participation.
